Position Summary

Family Practice & Counseling Network (FPCN), a program of Resources for Human Development, is a network of federally qualified community health centers that uses a nurse-managed, integrated model to deliver primary care, behavioral health, dental, and preventive services, which are co-located at the centers and are designed to treat the whole patient.

FPCN's Integrated Services & Digital Health Navigator will be based at the Abbottsford Falls location and work collaboratively with the shared services department's social workers and Community Health Workers to identify and establish relationships with high-priority patients, and help to identify digital and technological gaps that exist and to develop specific trainings for diverse populations to address low, intermittent and higher levels of digital literacy. The member will work directly with patients in the clinic and in their homes (as needed) to support engagement with technology including FPCN's patient portal, home health care devices such as blood pressure cuffs, and telehealth visits.

In conjunction with digital health navigation, the Integrated Services & Digital Health Navigator will conduct screenings with patients and community members for Social Drivers of Health (SDOH). Through these screenings they will identify additional barriers that prevent access to health care and support a decent quality of life such as housing stability, food security, transportation employment and linkage to community supports and resources.

Major Duties and Responsibilities

Digital Health Navigation:

  • Providing education support to patients on how to use technology to connect to care, including how to use remote monitoring equipment, how to participate in a telehealth visit, and how to use their patient portal.
  • Helping patients to set-up and test devices and technology.
  • Working with health center staff to identify patients considered a high priority due to a lack of digital access, health conditions, and/or participation in specific remote monitoring programs.
  • Tracking and managing all data needed to meet reporting requirements, including number of patients assisted, support provided, barriers encountered, and outcomes.
  • Supporting development of patient-facing education materials focused on healthcare technology and connecting to care.

Health Navigation:

  •  link individuals to both internal programming and external resources that are needed following their Social Drivers of Health Screening outcomes.

Knowledge Required for the Position

  • Knowledge of AmeriCorps/Health Corps member requirements
  • Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.

Supervision

  • Member uses initiative in carrying out recurring assignments following set procedures, independently
  • The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required

Review

  • Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.

Guidelines

  • Written and oral guides provide specific instructions for doing service
  • Guidelines are generally applicable, but the member independently makes adaptations in dealing with problems and unusual situations

Complexity
