Mature Health Connections

The member will collaborate with nursing to provide health education, screening, and community collaboration services to adults in Northwest Philadelphia. These include: member will meet with community leaders/LaSalle partners and groups to identify and collaborate on continued health promotion needs. Based on those needs, the member will plan and implement health education, screening, and immunization activities for the targeted community. This may include stress management, heart health, stroke awareness, and influenza outreach. Member will provide Mature Health Connections participants with appropriate individual health education, and referrals for follow-up to medical care, as needed. Member will assist Mature Health Connections participants with follow-up in-person and by telephone to attain medical and insurance appointments, and pharmacy refills.
Activities will occur on 2 days weekly for a total of 16 hours weekly.
