Service at The Health Annex:
Health and benefits advocates' primary activity is to conduct insurance assessments and help patients apply for health insurance through the Marketplace and the Department of Public Welfare. Patients work with health and benefits advocates to find prescription assistance programs that cover or subsidize the cost of their medications. Health and benefits advocates also help patients navigate the public welfare system and apply for other public benefits including cash assistance, food stamps, and temporary assistance for needy families. Health and benefits advocates also identify high-risk, chronically ill patients and connect them with existing self-management programs, including diabetes support groups and parenting groups.
