Care Coordinator

Position Summary

The member will be primarily responsible for Activate program patients. This position will provide ongoing support for the patient and team-based care model. The Activate Coordinator will work with Operations, Nursing and Quality Improvement personnel to disseminate and collect information related to patient care. The coordinator will work closely with care team members to assist with patient self-management, data collection, case management, continuity of care and referral coordination. The Activate Coordinator will be responsible for regularly reporting patient care-management data and findings with the relevant departments.

Major Duties and Responsibilities

  • Providing individualized, tailored care to a specific patient population
  • following up and assisting patients with social service and health care navigation
  • Recruiting, managing, and scheduling subject matter experts for patient education
  • Helping clients navigate the internet and with computer usage
  • Registering patients in the case management system,
  • Providing access to and awareness of resources for patients

Member uses initiative in carrying out recurring assignments following set procedures, independently. The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required. The supervisor assigns service activities in terms of project objectives and basic priorities and is available
for consultation in resolving controversial issues.


Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.


Guidelines are generally applicable, but the Member independently makes adaptations in dealing with problems and unusual situations. Member must use considerable judgment in adapting current or developing new guidance.


Member has to develop, analyze, or evaluate information before the service position can progress.
