Position Summary
The NHC Chicago AmeriCorps member's service will directly impact uninsured school age children and their families in the City of Chicago and neighboring suburbs. Through data capture, analysis and use of Social determinants of Health screening tool, the member will: Determine common diagnosis codes used to target services; provide follow up on referrals for additional clinical services and appropriate referrals to community based social services and supplemental programs such as Medicaid and SNAP; review existing food insecurity programs currently in use across all platforms of the School Health Program.
Major Duties and Responsibilities

Characteristics of an Ideal Candidate
We are a mobile clinic, and our AmeriCorps member will be connecting with families identified as needing referrals for services such as dental, PCP (Primary Care Provider), food, housing, vision services, clothing, and behavioral health. We have a database of Social Service Agencies the member can access as well as translation. Flexibility, empathy, and the ability to connect with families who might have different lived experiences is a must.

Knowledge Required for the Position
Knowledge of AmeriCorps/Health Corps member requirements; Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.
Supervision
Member uses initiative in carrying out recurring assignments following set procedures, independently. The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required. The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues. Supervision will be with a Master's trained Social Worker, and the supervisor looks forward to a close working relationship and growing together with the member.
Review
The Member independently plans and carries out the projects and selects the approaches and methods to be used in solving problems. Our current member is a self starter who meets with the supervisor weekly and other times as needed. The lines of communication are open and the member and supervisor are available for check-ins any time.
Guidelines
Guidelines are generally applicable, but the Member independently makes adaptations in dealing with problems and unusual situations. Referrals are made by the physicians and nurse practitioners seeing the students. The AmeriCorps member follows up with the family to offer services in the appropriate zip code, language and comfort of the family.
Complexity
The service position assignments vary significantly or often involve unusual circumstances and incomplete or conflicting data. Each family is unique and once contacted, may have needs the student did not tell the provider. The ability to communicate and connect important.

Member will need to pass the background check an drug screening by Advocate Children's Hospital and show proof of vacccines.
