Position Summary

The member will enroll and recruit Advocate Trinity Hospital’s patients to participate in the Healthy Living Program’s Food Farmacy. The member will coordinate food distribution and assess the program’s impact through several measures including qualitative and quantitative methods. The program directly aligns with NHC’s mission in improving access to health and education by providing direct services to those that are experiencing symptoms of diabetes, obesity and need health education to improve their quality of life, prevent diabetes, reduce obesity and other chronic conditions in the underserved in South Chicago.  The program specifically targets food insecure communities and those that are the most disparate in health outcomes. The program also focuses on connecting participants to health resources and other social determinants of health by improving access to identified services. The program also aligns with the NHC goal to increase physical activity and improve nutrition with the purpose of reducing obesity and/or chronic conditions that are linked to obesity.


Major Duties and Responsibilities

Engaging in outreach and promoting health education topics and healthcare services; Collaborating with pre-existing patients and partner organizations; Recruiting participants; Creating healthcare and wellness educational materials; Conducting data collection; Keeping and organizing documentation and records; Engaging in program planning; Doing physical exercise demonstrations; Coordinating with food access programs.

Knowledge Required for the Position

Knowledge of AmeriCorps/Health Corps member requirements; Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.


The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required.


The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the Member at service. Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.


Written and oral guides provide specific instructions for doing service. Most instructions are easily memorized and require little interpretation. Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards and files of previous projects.


The facts or conditions of the service position are clear cut, predictable, and apply directly to the assignment.
