Position Summary

This position will expand Erie’s service around social determinants of health by assisting patients with: screening for SDOH needs, providing resource linkages to patients, scheduling appointments, assisting with applications for benefits and pharmaceutical services, and more. The SDOH Outreach Worker will support patients across Erie and collaborate with teams across the organization.

Major Duties and Responsibilities

Creating and developing communications related to outreach campaigns; Obtaining feedback to improve on communication materials; Developing and providing health education for the community; Utilizing partnerships and data to coordinate referrals; Attending trainings and/or community events; Maintaining educational materials in waiting/examination rooms; Responding to and tracking requests through online services; Identifying new sites to provide resources and materials; Maintaining an inventory of supply materials for patients, clients, and/or organizations; Developing and distributing new materials geared towards patients, clients, and/or organizations; Developing and maintaining monthly inventory reports; Conducting enrollment benefits screenings; Linking clients from community organizations to services; Meeting with referral staff to ensure that clients receive services; Assessing and reporting on existing services in the target area; Networking with the community; Coordinating with outside facilities, faith-based organizations, social services agencies, private medical practices, etc.; Linking individuals and treatment services via education and by serving as a point of contact for community-based agencies; Collecting and tracking of data.

Knowledge Required for the Position

Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.

Supervision

The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required. The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues.

Review

The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the Member at service. Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.

Guidelines

Written and oral guides provide specific instructions for doing service. Most instructions are easily memorized and require little interpretation. Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards and files of previous projects.

Complexity

The facts or conditions of the service position are clear cut, predictable, and apply directly to the assignment. In making decisions, the Member is often required to depart from past approaches and to extend traditional techniques.
