Position Summary

The Central Neighborhood Community health Fellow will develop and implement an outreach plan for Central Neighborhood Health Foundation Clinic(s).  Member will provide outreach, health education, and navigation activities throughout the community.  Fellow will be responsible for coordination oof outreach activities and screening for Marketplace Insurance in-house and in the community.

Major Duties and Responsibilities

Member will plan and execute outreach and education activities in hours and in the community.

Member will build new partnerships with other community-based organizations.

Member will Attend appropriate community or networking meetings to facilitate outreach and gathering of information to support community access to healthcare.

Member will assist in the development, implementation, and maintenance of an Outreach and Education Program for Riverside Clinic

The ideal candidate for this position will be an outgoing individual who is passionate about serving the community.  We are seeking someone who is a self-starter who can build new programs and initiatives based on community feedback and collaborations.

Knowledge Required for the Position

•    Knowledge of and experience in developing, implementing, and working in a community outreach program

Knowledge of existing social service programs available In the Central Neighborhood Health Foundation service areas.

Proficient in use of computers and software programs such as Word, Excel, and PowerPoint; 

Experience in creating social media posts on Facebook, Twitter, Instagram, LinkedIn and be able to utilize the internet.
