What do you do at your site to lower health disparities for the community you serve?
I serve as a care manager at the Pittsburgh Mercy Family Health Center, which is a medical home that serves members of our community who face challenges of mental and behavioral health issues, homelessness, and drug addiction.   One project that I am very excited to be working on is to support and encourage patients to complete advanced directives, which gives patients the ability to state specifically what kind of care they want, in the event that they are unable to do so. As a result, patients are able to take control of their own healthcare.
 

What are you thankful for during this Thanksgiving holiday?
At the Pittsburgh Mercy Family Health Center, I serve with an amazing team that is committed to providing exceptional healthcare services to some of the highest need members of the community.  Day in and day out, I’m inspired and motivated by their unwavering commitment to serving our patients with compassion, love, and determination.  I am thankful for having the opportunity to be a part of such a unique and amazing group of people.
