Position Summary

 The NHC member position will improve the patient coordination, resources, and planning for community individuals served across the continuum of care.   The NHC member will also develop outreach methods with a focus on reducing health disparities and enhance the VNA’s range of services as well as expand existing services to new geographic areas.

 

 

Major Duties and Responsibilities

  • Developing new programs or services to address community needs
  • Modifying organizational systems/practices to improve service efficiency and/or effectiveness
  • Strengthening volunteer recruitment and/or retention efforts
  • Conducting research, mapping community assets, or reviewing data to strengthen the ability to meet community needs 
  • Developing/implementing surveys, focus groups, interview guides, observations, or other data collection tools to identify opportunities for improvement in service delivery
  • Engaging stakeholders to understand goals, needs, priorities, challenges, and opportunities 
  • Developing communications messages/strategies 
  • Creating tracking or data management systems

Knowledge Required for the Position

Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and work tracking options

Supervision

Member uses initiative in carrying out recurring assignments following set procedures, independently.

Review

The work is reviewed for accuracy through spot checks, through complaints from customers, and through observation of the employee at work.

Guidelines

Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards and files of previous projects.

Complexity

Member has to develop, analyze, or evaluate information before the work can progress.
