Community Health Worker

The Community Health Worker provides a bridge between patients at the Stephen Klein Wellness Center and their health care providers and other professionals for referrals, decision making and overall continuity of care. They also assist visitors to the Stephen Klein Wellness Center with their wellness needs and attempt to provide them support and referral to appropriate providers and/or agencies.

They work with Project HOME’s residential managers and case workers to connect residents to services at the Stephen Klein Wellness Center and our other two clinical sites. They help patients enroll in health insurance and accompany them to appointments, while also following up with patients about ways that they can improve their physical and behavioral wellness on their own. Community Health Workers participate in case review meetings and also assist patients who are in recovery groups like medication-assisted treatment for substance use.

Typically, Community Health Workers at Project HOME provide a bridge between patients at the Stephen Klein Wellness Center and their health care providers and other professionals for referrals, decision making and overall continuity of care.

This volunteer will serve in the position of a Community Health Worker. However, the work that they will do in this position will differ from our other CHWs. Instead of working exclusively with the existing patients at our health care center, this CHW will focus on engaging with people in the surrounding community who are not patients. They will be attending local events and going door-to-door to promote our services and educate patients on ways they can improve their physical and behavioral wellness on their own. This includes Connect4Health, which is an online directory of local resources that patients can access for free thanks to our partnership with the Philadelphia Health Federation.

This volunteer will also work with our partners at the Helen Brown Community Center, Honickman Learning Center and Sanctuary Farms, who run a community garden right across the street from our primary clinical site.

• Educating patients on medical interventions and prevention

• Maintaining, assembling, and distributing materials and supplies
