Care Coordinator

Position Summary

MEDICAL SCRIBE (90%)
Documentation: Member will accompany physicians during patient visits and document the encounter in real-time. This includes recording patient history, physical exams, and any other pertinent health information in the electronic health records (EHR) system.
Administrative Support: They assist with clerical duties such as preparing medical record documentation, organizing patient charts, and ensuring all documentation is complete and accurate.
Communication: Member will communicate with patients and healthcare providers to ensure smooth information flow. They may also handle patient calls and follow up on test results.

Medication Management: Under supervision by medical provider, member may assist in electronically prescribing medications and ensuring that all prescriptions are accurately documented.
Efficiency Enhancement: By handling documentation and administrative tasks, member enables physicians to spend more time on direct patient care, improving overall efficiency and patient satisfaction.

Patient Experience (10%): Shadow and observe LCH Clinical provider/patient relationship (i.e. documentation).

Major Duties and Responsibilities

Medical Scribe (90%)
Member will accompany healthcare providers during patient visits and document the encounter as it happens. This includes recording patient history, physical exams, assessments, and treatment plans in the electronic health records (EHR) system.
Member will handle clerical duties such as preparing medical record documentation, organizing patient charts, and ensuring all documentation is complete and accurate. Member will facilitate communication between patients and healthcare providers, ensuring smooth information flow. They may also handle patient calls and follow up on test results.
Under medical supervision Member may assist in electronically prescribing medications and ensuring that all prescriptions are accurately documented. By managing documentation and administrative tasks, member enables healthcare providers to spend more time on direct patient care, improving overall efficiency and patient satisfaction.

Patient Experience (10%)
Shadow and observe LCH Clinical provider/patient relationship (i.e. documentation). Gather information on how LCH can improve capture patient experience.

Knowledge Required for the Position

Medical Terminology - Preferred

Background in Public Health, Medical Office - preferred

Spanish
