Position Summary

The Harmony Health Outreach Community Health Fellow will support the work of Harmony Heath by increasing our ability to connect with, learn from, and provide services for our local community. 

The Fellow's first primary objectives will be to connect Harmony Health to external Community uplift initiatives by serving as the Harmony Health representative to the Yuba City Healthcare Council and the Yuba Community Blue Zone project to assess community needs and support Harmony’s participation in community-based solutions to local inequity, especially around issues of food access and nutrition. Our member’s second primary objective will be to support outreach initiatives by building community connections in previously identified target communities. The member’s third objective will be to direct and complete Harmony Health’s new STI training and testing initiative. 

During their time working on these projects, the members will serve the under- and uninsured population, individuals and families who are food insecure, formerly incarcerated, young people, and others at high risk of contracting and spreading STIs, non-English speaking populations, and immigrant and undocumented populations.
 

Major Duties and Responsibilities

This Community Health Fellow will support the following projects:

  1. STI training and testing initiative
  2. County Blue Zone Response—Food Desert and Nutrition projects
  3. Affordable Care Act enrollment and registration (formerly incarcerated population)
  4. Clinic on wheels, in order to build capacity and expand the impact of Harmony Health within in the larger community. 

Characteristics of an Ideal Candidate

We are seeking a candidate who is passionate about engaging with and learning from members of the community. An ideal candidate for this position would be an outgoing individual who has experience working with diverse communities and engaging in creative problem-solving with others.

Spanish language skills are a plus!

Knowledge Required for the Position

We are seeking a candidate who is familiar with the Yuba County (Marysville) community and has some knowledge about obstacles to care and engagement.

There is no requirement for previous experience in a health center setting.

Supervision

Alicia Aguirre, LVN has been with our organization for 5 years and served as Operations Manager for the past two years.  She is responsible for supervising all support staff for 4 clinics and the clinic on wheels. Alicia has aspirations to become an RN and is enthusiastic in mentoring others to reach their potential.  She has served as case manager for our MAT program, overseen our covid vaccine program and prenatal program has excellent time management, organizational skills and is diligent in documentation and reporting.

Language Proficiency Requirements

Spanish language skills preferred
