Position Summary
The member will support the Sullivan County Laurel Health Center, a federally qualified health center, located in rural PA. As an extension of the Laurel Health Center healthcare team, community health workers (CHW) assist in coordination of care and education, identifying individuals and families to assist in healthcare utilization. CHW’s contribute to improved health outcomes through patient outreach to enhance continuity of care and provide support in treatment plans for preventative care and disease management. CHW’s advocate, educate, and support patients in order to help them improve their lifestyle and connect them with appropriate healthcare options.
Major Duties and Responsibilities
-Bridge the gap between health care providers and patients by helping patients navigate the healthcare system, community resources and social support services.
-Establish and maintain trusting working relationships with patients and their families while providing general support and encouragement.
-Manage compliance of patient populations within assigned health center(s) to improve health outcomes and quality metrics.
-Help patients set and attend appointments; including coordinate of transportation services. Following up with patients via phone calls and visits.
-Support Provider and Patient Treatment Planning as a care coordinator supporting:
-Individuals missing age-based or seasonal screenings, tests, or vaccines.
-Preventative and Wellness Screenings
-Transitions of Care
-Quality of Care, and “Care Gaps”
-Build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and empowerment.
-Facilitates referral(s) and follow up services or otherwise coordinates care; and identifies and helps enroll eligible patients in federal, sate and local private and nonprofit health and human services programs.
-Maintain open communication lines with providers, patients, involved family, resources, etc to ensure service needs are achieved.
-Regular use of Electronic Health Record, navigate internal and external insurance plan(s) portals, databases, and spreadsheets.
-Capacity Building to Advance Community Health and Improve Health Outcomes- To be determined after collaborative listening session.
