Community Health Worker

The Community Health Worker (CHW) will be responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors.  In particular, the service member in this position will assist in outreach, education, and access assistance related to COVID-19 vaccination, testing, and treatment.

The CHW will support providers and case managers through an integrated approach to care management and community outreach.  As a priority, activities will promote, maintain, and improve the health of patients and their family. 

The service member will focus their work on rural Susquehanna County, which has approximately 17,000 community members.  The is a lack of access to primary and behavioral healthcare, as well as, food, housing, and transportation insecurity. 

 The CHW will support:

• Health Promotion & Disease Prevention:  Community Outreach, Home visitation for assessment of individual and family needs, Individual and group chronic disease support, linking individuals with community resources, assessing and assisting individuals and families with Social Determinates of Health disparities.
• ·Follow-up & Care Coordination:  Perform follow-up call and/or home visits, Link individuals to community and clinical services, assist in referrals and care coordination, assess root causes to help address social determinants of health.
• ·Support COVID-19 response:  Assist with scheduling individuals for vaccinations, testing, and treatments, arrange transportation of access to COVID-19 services, perform testing call back and education on COVID-19 precautions/isolation requirements, perform community outreach for vaccination promotion/education.
