Member will provide support to individuals eligible for HCV-curing treatment including outreach, engagement, follow-up, and celebration upon completion of treatment. Member will initiate contact with 10 individuals per month who are determined eligible for HCV treatment. Member will schedule and conduct reminder calls to patients for HCV treatment as they progress through the cascade of care.

Member will complete up to 5 prior authorizations for HCV-curing medications per month.
Member will organize individualized recognition and celebration of completion of treatment for 100% of individuals completing treatment during the service period. Member will initiate contact with 100% of individuals referred for HIV and PrEP. Member will schedule and conduct reminder calls to patients for monthly clinic sessions. Member will complete up to 5 prior authorizations for HIV-treating or preventing medications per month. Member will organize individualized recognition and celebration for 100% of individuals meeting care goals during the service period
