Position Summary

The Jefferson Collaborative for Health Equity is a broad-based community collaboration and action network that addresses health inequity challenges in Philadelphia.

Frazier Family Coalition serving as a wellness hub, the Frazier Center aims to serve as a connection point for health and wellness related activities. The Center also engages with educational programs and trainings for primary care physicians to better equip their practices to identify patients at risk for stroke and limb loss- both of which occur with alarmingly high frequency in our neighborhoods of focus.

The NHC member will be responsible for the day-to-day tasks associated with community based chronic disease self-management programming (hypertension, diabetes, stroke, etc.). They will serve directly with our team to maintain patient referrals, provide health education, information, and resources to the community benefit population served by Jefferson Collaborative for Health Equity (Collaborative).

The NHC member will participate in chronic disease self-management programs based on protocols and procedures developed by the Collaborative / Frazier Family Coalition. Tasks include data collection through surveys and interviews with program participants, data management and analysis, and generating reports.

Major Duties and Responsibilities

  • Engaging in outreach and promoting health education topics and healthcare services
  • Collaborating with pre-existing patients and partner organizations
  • Recruiting participants
  • Facilitating interventions and classes about health issue prevention and management and overall wellness
  • Facilitating and conducting healthcare and wellness trainings and classes
  • Assisting with program design
  • Opportunities for continuing education

Member will need a personal vehicle.
