Position Summary

Start Date is August 29th, 2022.

The NHC member will manage the All Love Team's list for clients to outreach, send out BOLO’s (Be on the lookout) for clients the team is looking for. The member will also review with NP, RN, or MD which clients need highest priority for outreach, attend weekly and monthly meetings, taking notes and a synopsis of the meetings. The member will also review EPIC and Emails for cc’d charts and referrals, review for closed cases and move cases.

The member would directly serve the homeless population, particularly women with their partners, anyone who identifies as a woman, women who are child bearing age and women who are pregnant or postpartum. 

Members will be serving the homeless population on a daily basis.

Major Duties and Responsibilities

  • Practicing intake and referrals of patients in need
  • Identifying patients who are at-risk to assess eligibility for various services
  • Offering and reviewing screenings or self-referrals
  • Referring patients to other programs and community services
  • Tracking patients to confirm program enrollment and engagement
  • Providing follow-up and case management to deliver assistance and information
  • Maintaining professional boundaries and respecting patient confidentiality
  • Educating patients on medical interventions and prevention
  • Documenting interactions with patients in an electronic health record
  • Attending and participating in community or host site events creating
  • Collaborating with supervisors and other leadership
  • Screening persons admitted for referral to behavioral health services conducting
  • Social determinants of health screener
  • Providing social service navigation
  • Maintaining, assembling, and distributing materials and supplies
  • Participating in meetings and conferences

  • Education in Social Work, Public health, and/or Psychology
  • Experience in interviewing or peer counseling
  • Comfortable speaking with patient populations, individually or in groups

Knowledge Required for the Position

  • Knowledge of AmeriCorps/Health Corps member requirements
  • Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options


  • Member uses initiative in carrying out recurring assignments following set procedures, independently.
  • The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required.
  • The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues.

Special Considerations

  • Special dress code: Our dress code is casual and comfortable, ex. jeans and sneakers due to the amount of outreach and working with the unhoused population, member's will be provided necessary PPE to perform their jobs and must be fully vaccinated for COVID and TB.

Language Proficiency Requirements

None, though Spanish, Cantonese, and/or Mandarin would be useful in supporting local communities with linguistic needs.
