Position Summary

This position is part of an ongoing effort for all homeless individuals who are in shelter/SIP sites to end health inequalities and end the opioid epidemic that has plagued this city. Member will be working with the most vulnerable homeless population on a regular daily basis, that are elderly, addicted to drugs, and have some mental health issues in the SIP Sites and Shelters that are ran by San Francisco Department of Public Health

SIP Site Coordinator – this team began during the COVID pandemic to assist nursing teams and health workers that are stationed in SIP hotels and congregate shelter in place sites.


Major Duties and Responsibilities

Major duties include:

  • Registration of residents at the sites who come into the onsite clinics
  • Assessing clients for needs, such as benefits, primary care doctors, etc.
  • Educating clients on Suboxone/methadone, and Narcan usage.
  • Inventory of OTC medications
  • Doing rounds in the sites with the nurses
  • Assisting clients in making appointments.
  • Practicing intake and referrals of patients in need
  • Identifying patients who are at-risk to assess eligibility for various services
  • Offering and reviewing screenings or self-referrals
  • Referring patients to other programs and community services tracking
  • Patients to confirm program enrollment and engagement
  • Providing follow-up and case management to deliver assistance and information
  • Maintaining professional boundaries and respecting patient confidentiality
  • Educating patients on medical interventions and prevention
  • Documenting interactions with patients in an electronic health record
  • Attending and participating in community or host site events creating
  • Collaborating with supervisors and other leadership
  • Screening persons admitted for referral to behavioral health services conducting
  • Social determinants of health screener
  • Providing social service navigation
  • Maintaining, assembling, and distributing materials and supplies
  • Participating in meetings and conferences

Depending on the status of COVID-19 in San Francisco, this position may involve responsibilities and duties around COVID-19 Response.

Knowledge Required for the Position

  • Knowledge of AmeriCorps/Health Corps member requirements
  • Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options
  • Member will be trained on using Epic patient database.


  • Member uses initiative in carrying out recurring assignments following set procedures, independently.
  • The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required.
  • The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues.
  • Member also can take the initiative to point out things that can be changed and different strategies for working with different clients.

Special Considerations

  • Special dress code: Our dress code is clean and comfortable due to the outreach portion of the job, usually jeans and sneakers are fine for the work we encompass.
  • Special safety regulations or precautions that must be observed
  • Health screening or service required: Each member should be vaccinated for COVID and have a TB clearance. All members are required to wear masks in all our facilities, unless you are eating and drinking.

While not required, it would be beneficial if candidates spoke one of the following languages spoken by local populations:
