Position Summary

Summary: The member will be a part of the care team on the medical Street Outreach Services (SOS) van and to support the Shelter Health team. Major duties in both programs include: traveling in medical van or shelter health clinic, outreach and screening of potential participants, supporting on-site preventive and urgent health care, providing connections to primary care, including creating and escorting clients to appointments, supporting linkages to services: mental health, substance use recovery, dental, social, and legal services, and working closely with the multidisciplinary teams and community based organization staff of relevant programs.
About the Organization(s): The SOS van provides outreach and medical services to people experiencing homelessness in specific San Francisco neighborhoods, while the Shelter Health clinics are located within the City's system of homeless shelters and Navigation Center. SFDPH Whole-Person Integrated Care serves to provide care to people experiencing homeless by 1) connecting to transitional primary care 2) re-/connecting to social services 3) supporting client’s towards becoming “housing ready” for their transition from shelter health to their next destination, ideally their own permanent, supportive housing with established primary medical care.
