About the Position: The WPIC Subtance Use Coordinator will support unhoused people by connecting them to substance use treatment to reduce the risk of overdose and improve their health. 

The member will support the INSPIRE Clinic, a multidisciplinary care team, including nurses, health workers, peers, social workers, and medical providers, to offer treatment to people with stimulant use disorder using a model called Contingency Management. The member will also help people with opioid use disorder start and stay on medication for addiction treatment through patient navigation, outreach, and linkage to services.

About the Organization: SFDPH Whole-Person Integrated Care serves to provide care to people experiencing homeless by 1) connecting to transitional primary care 2) re-/connecting to social services 3) supporting client’s towards becoming “housing ready” for their transition from shelter health to their next destination, ideally their own permanent, supportive housing with established primary medical care.  

Shelter Health is one of 8 social medicine programs under the Whole Person Integrate Care umbrella. This position would be a vital part of care coordination, stabilization, and wrap-around services to adults experiencing homelessness.

• The supervisor will meet with NHC members regularly to guide them and ensure they can complete their assigned activities.

• The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service.
• The supervisor will explain the project guidelines and offer instructions on completing them.

• Written and oral guides provide specific instructions for doing service
