Project TALKS – Talk, Act, Listen, Know Support

The concept of Project TALKS – Talk, Act, Listen, Know, Support - is to create talkable communities. This is the first program of its kind in the state of Florida to leverage the power of 5 major behavioral health providers (SPBH, Gateway, Child Guidance center, Clay behavioral, EPIC) across 6 counties (Nassau, Duval, Clay, S. Johns, Putnam, Flagler) to spread the message of attainable mental wellbeing to Northeast Florida. Our project creates trauma informed communities that will encourage healthy conversations, activities and interventions that we believe will spearhead a focus on mental well-being leading to happy, healthier lives. Project TALKS goal is to create talkable communities that foster improved physical and mental well-being, increase social connectedness, decrease the incidence of MH crisis and effectively address trauma.

Activities/services

  • Create talkable communities throughout NEFL to increase the number of individuals with the ability to respond to a mental health Crisis. (TALK)
  • Expand care coordination activities in local hospitals to connect with those experiencing a mental health crisis (ACT)
  • Develop a partnership with first responders that integrates a MH professional into their response team and that offers trauma support to first responders (LISTEN)
  • Increase referrals to Mental Health services and other community resources (KNOW)
  • Provide trauma support to family and friends who have lost someone due to suicide and/or overdose (SUPPORT)

Starting Point is looking for an NHC member to provide direct service to this vulnerable population by assisting in the following ways: 

  • TALK - The NHC member would be trained in Adult/Youth Mental Health First Aid and Teen Mental health First Aid to assist in then providing this training to groups of adults and teens. The NHC member would also assist with suicide prevention presentations in schools and the community. The NHC member would preferably have marketing skills to assist SPBH in promoting Project TALKS.
  • ACT - The NHC member would work and collaborate with our Care Coordination department to receive referrals from community providers such as the Emergency Department, Crisis Stabilization Units, and the Mobile Response Team. 
  • LISTEN - The NCH member would maintain a small caseload to provide case management services to current clients engaged in Project TALKS to help facilitate treatment and promote stabilization.
  • KNOW - The NHC member would reach out to appropriate referrals to help reduce barriers to entering specialized treatment.  The NHC member would also help assist with any discharges to ensure clients are prepared with resources they may need in the future. 
  • SUPPORT - The NHC member would assist with providing support groups for survivors of suicide and Teens experiencing increased stress and anxiety due to COVID-19.

Major Duties and Responsibilities

  • Identifying patients who are at-risk to assess eligibility for various services
  • Referring patients to other programs and community services
  • Tracking patients to confirm program enrollment and engagement
  • Providing follow-up and case management to deliver assistance and information
  • Implementing patient engagement strategies to improve outcomes
  • Developing and/or facilitating classes with other professionals
  • Maintaining professional boundaries and respecting patient confidentiality
  • Coordinating health and wellness activities including classes and groups
  • Documenting interactions with patients in an electronic health record
  • Attending and participating in community or host site events
  • Collaborating with supervisors and other leadership
  • Implementing strategies to recruit clients to attend health and wellness classes
  • Screening persons admitted for referral to behavioral health services
  • Conducting social determinants of health screener
  • Participating in meetings and conferences

Knowledge Required for the Position

Knowledge of AmeriCorps/Health Corps member requirements. 

Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.

Supervision

The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues.

Review

Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.

Guidelines

Written and oral guides provide specific instructions for doing service.

Complexity

Member has to develop, analyze, or evaluate information before the service position can progress.

Criminal History Check Requirements Beyond NHC Standard Checks

Starting Point Behavioral Healthcare is required by the Department of Children and Families and the Agency for Healthcare Administration to perform level 2 background screening checks on all employees and volunteers. All applicants must pass the background screening or receive a waiver from DCF. An applicant with a criminal charge on the list of offenses defined by DCF is not eligible to apply.
