Position Summary

About the Position:

The Food Security Program Assistant will strengthen and support Harmony Health's food security initiatives as part of the Food Security Program (FSP). The NHC Member will provide nutritionally adequate food to Harmony Health patients and individuals experiencing homelessness, connect patients to and reduce barriers in accessing community-based food and nutrition resources, and deliver tailored health education on nutrition incentives, food utilization, and lifestyle management to support health outcomes.

The NHC Member will manage the food pantry programs, including inventory and outreach, facilitate gardening activities for patients, and provide patient education around CalAim services and eligibility for nutrition benefits.

The NHC Member will serve individuals across Yuba and Sutter counties and will be working across multiple teams and levels at Harmony Health to improve and implement services for all patients.

Their medical clinic provides prenatal care and is the only Comprehensive Perinatal Services Program in Yuba County. Harmony Health's integrated Behavioral Health services include Medicated Assisted Treatment (MAT) for Opioid Use Disorder and pain management modalities including Cognitive Behavioral Therapy, Bio/Neuro Feedback, Acupuncture, Chiropractic services, meditation classes and substance abuse support groups.

Major Duties and Responsibilities

The NHC Member will complete the following responsibilities including, but not limited to:

  • Developing and providing health education to the Host Site’s patient population
  • Providing health education to patient populations
  • Attending trainings and/or resource events
  • Maintaining an inventory of supply materials for patients, clients, and/or organizations
  • Teaching classes/workshops in-person and remote on health
  • Developing/implementing surveys, focus groups, interview guides, observations, or other data collection tools to identify opportunities for improvement in service delivery
  • Developing and/or providing staff education or training

Special Considerations

  • Schedule
    • The member will:
      • Serve approximately 40 hours per week.
      • Serve Monday-Friday from 8:00AM - 5:00PM.
  • Dress Code:
    • Business casual, no open toed shoes within the clinic space or pantry space. Slacks, Chino pants, blouse, simple shirts (no messaging or statements), or polo shirts. Fridays we are allowed to wear jeans, but they cannot have any holes in them.
  • Personal Vehicle use:
    • This position requires the use of a personal vehicle. Mileage reimbursement is available. A personal vehicle is required because many homeless patients are in hard-to-reach areas. Some days commuting between clinics may be required.
  • Unusual Physical Demands:
    • Food pantry tasks include lifting up to 50lbs, reaching, and stooping.

Language Proficiency Requirements

  • Spanish – Limited Working Proficiency is Required. Harmony Health defines their need as follows:
    • Speaking - Limited Working Proficiency: Able to satisfy routine social demands and limited work requirements.
    • Reading - Limited Working Proficiency: Able to read simple prose, in a form equivalent to typescript or printing, on subjects within a familiar context.
  • Applicants with Basic Proficiency and a willingness to improve their language skills to better serve patients are highly encouraged to apply!
